Case Manager

Status: Non-Exempt  

Reports to: Director of Programs

Position Summary:

We are seeking a Case Manager-Bilingual (Preferred) in the Mid-Wilshire area to provide job placement services and job-site based support, supervision and instruction to participants with intellectual & physical disabilities. The Case Manager will also serve as one the primary points of contact for the individuals receiving supported employment services, their families, Regional Center, and other community service providers involved in the individual’s cases, and employers within the community. 

Education:

Bachelor’s Degree preferred and/or at least 3 years of related work experience in case management and job development preferred. Proficiency in written and verbal English required, bilingual (English/Spanish) skills preferred. DSP1 & DSP2 certification required. 

Essential Functions:

  • Develop, cultivate and maintain business partner relationships

  • Present employment model to employers

  • Negotiate new or renewal wage level with employers

  • Accompany participants on their first day of work

  • Create, complete and maintain all required documentation and reports

  • Attend ISP meetings, as assigned

  • Provide interview training to participants

  • Identify and engage in network-building activities (e.g. community and professional organizations)

  • Ensure and comply with all Liberty Employment Services, Inc. policies and procedures

  • Ensure that confidentiality is followed as prescribed by Agency policy and Federal and State Laws for the individuals we serve and support

Additional Essential Functions:

Experience: · Experience with job development and placement strategies, including: job analysis, task analysis and job carving · Familiarity with job coaching support, instructional strategies and developing on the job natural supports · Experience with instruction in critical life need areas, with emphasis on employment readiness, job placement and employment · Experience with small group placement · Direct (hands-on) experience with individuals with a wide range of intellectual & physical disabilities, mental illness, substance use, including persons with the most severe challenges · Experience in the community-based instruction in critical life needs areas · Experience in assisting individuals in the use of adaptive devices and communication aids · Participates in any agency required training. · Serve as a role model for Liberty Employment Services, Inc. staff promoting the agency Mission, Vision, Values and Guiding Principles. · Foster agency and program wide cooperation and teamwork through use of positive / constructive communication techniques. · Ensure and comply with all Liberty Employment Services, Inc. policies and procedures · Ensures that confidentiality is followed as prescribed by Agency policy and Federal and State Laws for the individuals we serve and support. · Provide case management for an assigned caseload who receive supported employment services within each individual’s plan of care. · Develop a service plan designated for Liberty Supported Employment and/or Liberty Employment Services that meets the needs of the individual on your caseload and acts as the liaison between both programs as needed to support the individual. · Work in conjunction with job coaches and management team to ensure that the individuals on each case load find employment and have meaningful employment skills. · Ensure that all individual case files and other documentation (timesheets, Therap documentation, and other forms) are organized and maintained at all times. · Ensure that daily contact and/or interaction with Regional Centers, individual employers, and Liberty Management Team meet the mission and vision of Liberty Employment services. · Ensure that all ISP meetings are scheduled, attended, completed and submitted within 30 days, as assigned · Other duties as assigned 

Required Skills:

  • Ability to communicate effectively both orally written

  • Proficient in English writing ability; including usage, grammar, spelling, punctuation and vocabulary

  • Ability to make effective presentations

  • Proficient in basic office equipment, including Microsoft Office Suite

  • Ability to teach and demonstrate job search skills

  • Ability to effectively negotiate wage levels

  • Knowledge of benefit counseling (SSI, SSDI)

  • Marketing and\or sales experience

Additional Required Skills:

Other Requirements: · Immediate access to a vehicle with working seat belts, valid driver’s license, current auto insurance and acceptable DMV record · Ability to travel within and throughout the assigned program area in which services are provided · Ability to travel in and throughout the county on a quarterly basis to attend meetings, training, etc. · Successful completion of drug screen and background check. · CPR & First Aid \ CPI (crisis prevention intervention) is required within the first 30-60 days of employment. 

Compensation:

Actual wage is based upon level of education and experience.
